Scenario

In a store, a man suddenly clutches his chest and collapses, showing signs of severe chest pain. How should you respond?

1. Scene safety

  • Verify the scene is safe for both you and the patient.
  • Check for responsiveness by tapping the shoulders and shouting.

2. Breathing & Pulse

  • Scan the chest for breathing and palpate the carotid pulse.
  • Check for no less than 5 but no more than 10 seconds.

3. Emergency Activation

  • If unresponsive, activate the emergency response system (call 911) immediately.
  • Send a specific bystander to retrieve an AED.

4. Positioning

  • Place the patient supine on a firm, flat surface.
  • Position the heel of one hand on the lower half of the sternum.
    (CRITICAL: Avoid placing hands too low. Compressing the xiphoid process can cause internal injury)
  • Keep elbows locked with shoulders directly over your hands.

5. Start Chest Compressions

  • Deliver compressions at a rate of 100–120 bpm (about 2 per second).
  • Compress at a depth of 2–2.4 inches (5–6 cm).
  • Allow full chest recoil; do not lean on the chest.

6. Give Breaths & Continue CPR

  • Perform a head-tilt/chin-lift to open the airway.
  • Deliver 2 rescue breaths (1 second each) with visible chest rise.
  • Maintain a ratio of 30 compressions to 2 breaths.
